Typical Insurance Requirements
WC: $1M minimum; exemption only if contract < $250K (per FL statutes).
GL: $1M; US Construction Corp. as additional insured.
Bodily Injury / Property Damage: $1M.
Personal Injury: $1M.
Umbrella/Excess Liability: $1M.
COI: Original required before project start.
License Requirements
Occupational License: Copy required for our records.
State Contractor License: Must provide valid license number for the state of incorporation.
Federal Tax ID: Required for all subcontractors.
Business Requirements
Completed W-9 or Substitute 1099 Form.
Bonding Capability: Indicate if your company is bondable and provide bonding company details.
Experience & Capacity
Minimum years in business and proof of relevant trade experience.
Provide EMR (Experience Modification Rate) for the last two years.
References
At least 3 project references from the past 3 years (preferably restaurant/retail projects).
